1. Save the PDF form to your computer.
2. Open the form with Adobe Reader. Fill it in and save it.

Forms requiring an original signature

To complete the Part-time Application, Spousal Consent and Declaration form and Cheque Replacement Statutory Declaration:

1. Print off the completed form and provide an original, ink signature.
2. Scan the signed form and check to ensure all form fields are complete.
3. Login to your account and submit securely using Upload eDocuments.
